Résumé: Les compressions vasculaires de la trachée entraînent classiquement un stridor ou un wheezing persistant aggravé par les cris, l'alimentation et la flexion du cou. Observations. - Trois enfants âgés respectivement de 3, 5, et 2 mois ont été hospitalisés pour un tableau de bronchiolite aiguë. Le premier avait un wheezing intermittent depuis 1 mois; la radiographie du thorax de profil a montré une opacité entre la partie basse de la trachée et l'oesophage. Les examens complémentaires ont montré une artère pulmonaire gauche de trajet aberrant, entraînant une sténose de la trachée qui a été ensuite opérée avec succès. Le deuxième malade a présenté une atélectasie pulmonaire qui a nécessité une bronchoscopie; celle-ci a montré une sténose trachéale dont l'investigation a révélé qu'elle était due à une artère pulmonaire gauche de trajet anormal. La troisième enfant avait des antécédents de stridor congénital; la détresse respiratoire a nécessité une intubation endotrachéale et l'existence d'une atélectasie pulmonaire a fait faire une fibroscopie; celle-ci a montré une sténose de la trachée qui est secondairement apparue étre en rapport avec un double arc aortique. La situation a été améliorée après correction chirurgicale. Conclusions. - Les antécédents de stridor et/ou de wheezing chez des enfants hospitalisés pour bronchiolite doivent faire envisager 1a possibilité d'une compression vasculaire de la trachée et faire réaliser les investigations nécessaires.

Abstract: Background. - Anomalies of the aortic arch and pulmonary arteries may produce compression of the trachea with chronic stridor or wheezing aggravated by crying, feeding and flexion of the neck. Case reports. - Three infants, 3, 3 and 2 months-old, respectively, were admitted suffering from acute bronchiolitis. The first patient had intermittent wheezing for one month; lateral chest X-rays showed an opaque area inserted between the trachea and oesophagus and computed tomography showed pulmonary artery sling with tracheal stenosis which was successfully operated. The second patient displayed pulmonary atelectasia which required bronchoscopy; this technique showed tracheal stenosis which was due to pulmonary artery sling. The third patient had a history of congenital stridor; he required endotracheal intubation; a bronchoscopy performed because persistent pulmonary atelectasic showed tracheal stenosis which was secondary to a double aortic arch. The condition was improved after surgical correction. Conclusions. - A history of stridor and/or wheezing in patients admitted for bronchiolitis must lead to consider the possibility of vascular compression of the trachea and to perform specific investigations.
